Senior Administrator, Population Health, Weill Cornell Medicine

Semhar is Senior Administrator of Population Health at Weill Cornell Medicine/Physician Organization. In this role, Semhar regularly interfaces with commercial as well as government payor systems, physicians and patients, and works to implement Value Based Care contracts, and large scale population health initiatives, with the aim of reducing healthcare costs and providing quality care to various patient populations. Additionally, Semhar manages the day to day activities of a Care Management team comprised of Registered Nurses, Pharmacists, and Social Work, to provide inter-disciplinary support to recently discharged patients, as well as those with multiple chronic conditions and at high risk for hospitalization. Prior to this role, Semhar was Senior Director of a non-profit organization, overseeing CMS care coordination programs targeted to Medicaid recipients, with the aim of addressing drivers of utilization and frequent emergency department and hospital admissions.
